  Change current advisor information or report new advisors to Headquarters.

• Membership Revocation Form
  Chapter advisors ONLY may complete this form TO OFFICIALLY REMOVE FROM MEMBERSHIP any Phi Theta Kappa member who does not meet the academic maintenance requirements for continuing membership in the Society as outlined in the local chapter's bylaws (approved by Headquarters) and in the International Constitution (Article IV, section 3).

• Membership Transfer Form
  Chapter advisors ONLY may complete this form, which allows eligible members to transfer their Phi Theta Kappa membership.
  

• Replacement or Correction Certificate Request Form
  Members or chapter advisors may use this form to request a replacement or correction certificate for members already reported to International Headquarters.

What is a PDF?

• Membership Application Form
  Application must be completed by all new members. Information from the Membership Application Form should be submitted to Headquarters, by chapter advisors ONLY. Access to this on-line form allows advisors to download the form and make photocopies for their membership invitations rather than requesting forms from Headquarters if they so desire.

NOTE: The official Phi Theta Kappa Membership Application Form must be completed by all new inductees. The Membership Application Form may not be altered in any way.
